If any trends are discovered, use form 10.2 to decide on further actions to be taken, to either eliminate or minimise the risk. Fill in the number section with a different colour or design to help determine the period. The injury chart is for the injuries suffered due to incidents, whereas the risk chart is to be completed for the machine or process which caused the incident (e.g., manual handling, grinder, etc.)
